Care Coordination
It's important to have a plan when the health and welfare of a loved one are involved. A geriatric care manager or social worker assesses the person's mental, physical, environmental, and financial conditions; prepares a plan of care tailored to the specific needs of the patient, and coordinates the services of appropriate providers.
Examples of services that may be required while living at home:
- Meal preparation
- Oxygen delivery
- Transportation
- Physical therapy
- Legal assistance
